Why we do it

The current model of healthcare was developed at a time when the main challenge was to provide hospital based care for acute conditions. Now, over 45% of the population has one or more long term conditions. In addition, we have an ageing population.

Both challenges require a different approach. A focus on early intervention and a greater emphasis on home based, personalised care also means that the users of care need to be far more involved, alongside professionals, in the design of services.

The economic situation increases pressure to ensure that standards of Quality, Innovation, Productivity and Prevention (QIPP) agenda are improved upon, even as costs are cut. All of which cannot be delivered without a radical change.

However, the NHS, local authorities and other organisations often struggle to develop and disseminate innovative solutions in a sustainable way and at significant scale.

The methods, tools, evaluations, metrics and partnerships we have developed - and continue to refine as we learn - addresses these challenges.
